Office Policies
Once you are an established patient with our office, you will receive an email invitation to become a member of our secure, encrypted patient portal. Once registered, you may request an appointment via email or speak to a member of our staff!
We at Lawrenceville Family Practice have extended our office hours Monday thru Thursday to better meet the needs of our community.
We strive to meet your expectations by offering same day appointments! However, adult and child physicals and adult pap smears must be scheduled at least 3 days in advance, in most cases.
If for any reason you cannot make your scheduled physical or pap smear, please contact our office 24 hours in advance to cancel or reschedule your appointment. Failure to do so will result in a $45 fee that will be charged to your account. This procedure has been put into place to limit the amount of “no show” wellness exams so that we can provide sick patients the opportunity to be seen by their physicians.
Prescription Refill
We ask that you be responsible for anticipating the need for refills. Requests may take up to 72 hours to process. Please allow 7-10 days for all prescription refills. You can request a refill via our secure, encrypted patient portal or via voicemail.
We don’t call in most medications, as this is not a good practice. We DO NOT call in narcotics. If you think you need treatment for a problem, come in and let one of our providers examine you.
Assignment of Benefits and Financial Policy
In Medicare/other insurance company assigned cases, Lawrenceville Family Practice agrees to accept the charge determined of the Medicare/other insurance company as the full charge and the patient is responsible only for the deductible, co-insurance, and non-covered services. Please understand that you are personally responsible for all fees regardless of insurance coverage. It is customary to pay for services when rendered unless other payment arrangements have been made in advance with the billing department. Also, any bills over sixty (60) days old will be charged a ten (10) dollar re-billing fee and every month thereafter that it is not paid. All deductible and co-payments are due at the time of service. In addition, any x-ray copies that you need a copy of will be an additional ten (10) dollars per CD.
Also, copies of all medical records per patient will be a flat rate fee of twenty-five (25) dollars. Effective November 23, 2009, there is a standard fee of twenty-five (25) dollars to complete general forms, excluding sports physicals and immunization forms.
If for any reason you cannot make this appointment, please contact our office 24 hours in advance to cancel or reschedule your appointment. Failure to do so will result in a $45 fee that will be charged to your account. This procedure has been put into place to limit the amount of “no show” wellness exams so that we can provide sick patients the opportunity to be seen by their physicians.
Billing Policies for Routine Physical Exams
Your insurance company may or may not cover a physical exam. Please consult with your insurance company prior to your scheduled appointment regarding payment for your physical.
If your insurance company does cover a physical exam annually, please ensure that it has been at least 365 days since your last physical exam.
